At Workmen’s Circle MultiCare Center, we recognize that addressing emotional and social well-being is essential for comprehensive care. As part of our commitment to holistic healthcare, we conduct thorough social histories and psychosocial evaluations for each new resident during the admissions process. Our dedicated team of Social Workers is available to support residents and their families in adjusting to life at our facility, offering guidance and assistance as needed.
Through educational initiatives, we empower residents and their families to understand resident rights, make informed healthcare decisions, and engage in advance planning for their care. Additionally, our Social Workers collaborate with residents and their families to explore discharge options and ensure continuity of care upon transitioning back to the community. We also provide follow-up support to discharged residents, ensuring they have access to necessary services and resources to support their ongoing well-being after returning home.
